After reading the following directions and general information, please return your completed application materials to the Admissions Office at the address below. Please note the MSIH is approved by the U.S. Department of Education for participation in federally-funded student loan programs.

Please complete the following:

Return the essay section of the application in single-spaced, typed form;
*
Essay question #1 should be no longer than 1 page in length; other essay questions should be no more than 3/4 page in length;
*
Type your name, address, social security number/identity card number, undergraduate institution, date of MCAT exam, and email address on an index card;
*
If available, please indicate your class rank or standing in your graduating class in the space below your grade point average;
*
Affix a recent passport sized photo of yourself on the front of your application;
*
Please have your transcript(s) and medical college admissions test (MCAT) scores sent to the MSIH Admissions office or email the sixteen digit verification code used to obtain your MCAT score report so we can print out your test score. The Admission Committee will not evaluate your application until it receives a complete transcript(s) of your academic work.

After completing the application, send it to the address below with a check/money order drawn on a U.S. bank for $85.00 (U.S.), payable to BGU-CU MSIH (Ben-Gurion University-Columbia University Medical School for International Health).
